The what, where, why, how and who’s of

Specialist Support Coordination

At Communication eXtra, we offer comprehensive support throughout your NDIS journey. We understand the unique needs of the Disabled and Deaf community, and we’re here to ensure that navigating your NDIS Plan is both accessible and empowering.

What is Specialist Support Coordination?

This is where a participant has additional, high or complex needs and circumstances, in addition to their primary disability.

These additional circumstances may include:
• Additional or complex disabilities and/or health issues
• Moving to independent living
• Guardianship situations
• Child protection and custody issues
• Justice issues (criminal court, corrections, prison)
• Mental Health services
• Housing or debt crisis
• Other high-risk factors in the participant’s life

Why do we need Support Coordination?

Communication eXtra will work with case managers and other professionals to ensure that the participant gets the best and most appropriate support possible.

If a participant has Specialist Support Coordination funds in their Plan, the Support Coordinator will work with the participant and providers and/or government agencies to determine which services will be charged as Support Coordination, and which services will be charged as Specialist Support Coordination.

Communication eXtra are available to be sub-contracted to an existing Coordination of Supports provider, to provide a Specialist Support Coordination service alongside the regular Coordination of Supports.
