Not sure what you will need ready for your Enquiry?

To set up your client file, we will need some information from you:

  1. A copy of the NDIS Plan (current plan)
  2. Disabilities on Plan
  3. Disabilities not covered by NDIS
  4. Email and phone number
  5. Plan Manager details if funds are plan managed
  6. Preferred contact method (SMS, email, video)
  7. Preferred video method (FaceTime, Zoom, Skype, FB Messenger etc)
  8. Please advise how participant communicates.  (Auslan, lip reading, verbally etc)
  9. Name of an emergency contact person for participant and their relationship to the participant and contact details
  10. Is the emergency contact Deaf or Hard of Hearing?
  11. Is the participant fully vaccinated for Covid 19?
  12. The name of preferred interpreter or interpreting service (if ever we need to make a booking for you)
  13. Some suitable dates/times, and location, for registration meeting.

If you can provide these by email as soon as possible, we will schedule a registration interview.
