Not sure what you will need ready for your Enquiry?
To set up your client file, we will need some information from you:
- A copy of the NDIS Plan (current plan)
- Disabilities on Plan
- Disabilities not covered by NDIS
- Email and phone number
- Plan Manager details if funds are plan managed
- Preferred contact method (SMS, email, video)
- Preferred video method (FaceTime, Zoom, Skype, FB Messenger etc)
- Please advise how participant communicates. (Auslan, lip reading, verbally etc)
- Name of an emergency contact person for participant and their relationship to the participant and contact details
- Is the emergency contact Deaf or Hard of Hearing?
- Is the participant fully vaccinated for Covid 19?
- The name of preferred interpreter or interpreting service (if ever we need to make a booking for you)
- Some suitable dates/times, and location, for registration meeting.
If you can provide these by email as soon as possible, we will schedule a registration interview.
